Weekend Warrior Sports Tournaments Inc. (WWST)

Athlete Liability Waiver & Release Form

By agreeing and participating in any WWST event, I acknowledge and agree to the following:

Assumption of Risk:

I understand that participating in sports tournaments and recreational activities involves inherent risks, including but not limited to physical injury, illness, property damage, or even death. I freely and voluntarily assume all such risks, whether known or unknown, even if arising from the negligence of Weekend Warrior Sports Tournaments Inc., its staff, volunteers, or partners.

Release of Liability:

I hereby release, waive, and hold harmless Weekend Warrior Sports Tournaments Inc., its directors, officers, employees, volunteers, sponsors, and event facilities from any and all claims, liabilities, demands, actions, or causes of action arising out of or related to any loss, injury, or damage that may be sustained by me while participating in or attending any WWST event.

Medical Treatment:

In the event of an injury or medical emergency, I authorize WWST staff or volunteers to seek medical assistance on my behalf. I assume full responsibility for any resulting medical costs.

Personal Responsibility:

I certify that I am physically fit to participate in sports activities and have not been advised otherwise by a medical professional. I agree to follow all rules and conduct myself in a sportsmanlike manner.

Media Release:

I grant WWST permission to use photos, videos, or recordings of me from events for promotional purposes, including on social media, websites, and marketing materials.

Binding Agreement:

I have read and fully understand this waiver. I understand that by signing this, I give up substantial legal rights, including the right to sue. This waiver is binding on me, my heirs, executors, administrators, and assigns.
